描述男主角成為監獄裡的醫療科長後，孤注一擲成長為掌握名牌大學醫院理事會的醫生的復仇故事。南宮珉飾演醫生「羅以帝」，他原本是大學醫院急診中心裡的王牌，醫術高超、充滿人性，無論病人的身分地位如何，他絕不會差別對待。但在醫院理事長的橫行霸道下，他因為醫療事故被吊銷醫生執照，之後考取了內科醫生執照，並自動申請到監獄的醫療科。那裡有全韓國「最熱門」的罪犯──總統、財經大亨、嗑藥藝人、賭博體育明星等，他將一一涉獵，並計劃自己的謀求之路。
