we don't know the true story behind it.  見習醫生 aka junior doctor doesn't have the ability to determine what kind of illness orange's dad has.  maybe it's the licensed medical doctor being lazy ...

not sure how things work in your country but junior doctor is not allowed to say your dad has 肺朥 under both hk and canada rules.  the final decision is determined by a licensed medical doctor.  ...
