Maybe i can share my experience here , b'cos i also iphone have appear safe mode before
I am using Iphone 3GS, after i have install BiteSms 5.0 Cracked.
Everytime when have phone come in, it will automatic going to safe mode and before that it will ask u restart your springboard. After i remoce BiteSms 5.0, don't have this problem anymore.

015135, Maybe you have install some application that is not suit for ur iphone version so it will go into safe mode. Maybe u can think what application after you have install n appear safe mode.
