need help with my usb pendrive

Hi, i have this 256 gb usb drive.
i tried to format it into NTFS and i done it.
once i done it and tried to copy some files in there which is still ok.
but for the second time i tried to open it and it freeze the My Computer and all the window explorer.
and now i tried to format it back to FAT32 but when i plug it into the usb port and it take long time to responds and when i open My Computer and right click and its freeze. and My Computer goes not responding.
i tried using the window disk management, disk wipe, cmd.exe, hp usb, tried different computer.
all not working..
need some serious help.
